Welcome to another semi monthly (well sort of semi monthly) newsletter. Medicare has announced several proposed changes which may impact your bottom line, so be sure to check out those articles. As always, if there is something you would like us to cover, just give us a call or send us an e-mail.

    Last month Juls and I attended the Washington State Fire Chiefs Conference in Wenatchee. We had a great time catching up with our clients and, in some cases, putting a face with the telephone voice. Thanks to all of you who stopped by to say hello.

    I would also like to take this opportunity to thank Poulsbo FD #18 for allowing me to come by last week to talk about some specific documentation challenges and how they related to reimbursement rates. At Systems Design, NW our job is to help you get reimbursed for the services you perform and to help you remain compliant with Medicare regulations. Thanks to Chief Shields and Deputy Chief O'Donohue for recognizing it's a team effort.


    Medicare Announces Hold On Payments

    Earlier this summer Medicare announced that they would be placing a "brief" hold on all claims set for payment during the last nine days of the Federal Fiscal year; September 22, 2006-September 30, 2006. Claims which were due to pay out during this time will be paid on October 2, 2006 with no interest or late payment penalties paid to the provider of service. What this means to you depends mostly on your volume of transports. Many of you will not notice the nine day hold while some of our smaller districts are more likely to feel the pinch. If your district depends on those weekly Medicare checks our suggestion is to get your August and early September MIRs submitted early. Remember that there is a 14 day payment hold for all electronically submitted claims and if that 14 day hold expires during this newly enacted nine day hold period you may even end up waiting as much as 23 days for claims to be processed.


    Medicare Proposed Changes

    Several of our employees recently attended a PWW audio conference on the recently proposed ambulance reimbursement changes. One of the proposed changes involves the urban/rural designations for mileage bonus eligibility. Currently these designation are based on numbers from the 1990 census and significant population shifts have occurred between 1990 and 2000.     Another important proposed change involves the current "Emergency Response" definition. Currently for response to be "emergent" it must first go through a 911 type dispatch system and the responder must take the immediate steps to get there. The proposed change involves hospital to hospital transfers. CMS has stated "in most of these cases, the beneficiary must be stabilized prior to the transport. Therefore, the need to maintain a state of readiness to respond immediately to an urgent call, warranting a higher emergency response payment, does not appear to be applicable to these situations." We will of course be following these proposed changes and will be addressing any revisions in following newsletters.

    Kim joined our staff three years ago and brought with her a wealth of billing experience. She is the Account Manager for Snohomish County FPD #1 and Clallam County FPD #2. In her "spare time" Kim enjoys spending time with her daughter Ashley, remaining involved with her church activities and is currently working to complete her externship hours to become a licensed mental health counselor.


    On the Lighter Side
    Old Fire Truck

    One dark night outside a small town, a fire started inside the local chemical plant. Before long it exploded into flames and an alarm went out to Fire Departments from miles around.

    After fighting the fire for over an hour, the chemical company president approached the fire chief and said, "All of our secret formulas are in the vault in the center of the plant. They must be saved! I will give $50,000.00 to the fire crew that brings them out safely.

    As soon as the chief heard this, he ordered the firemen to strengthen their attack on the blaze. After two more hours of attacking the fire, the company president offered $100,000.00 to the engine company that could bring out the company's secret files.

    From the distance a siren was heard and another fire truck came into sight. It was the local volunteer fire company, composed entirely of men over 65. To everyone's amazement, the little fire engine raced through the chemical plant gates and drove straight into the middle of the inferno.

    In the distance, the other firemen watched as the old timers hopped off their rig and began to fight the fire with an effort that they had never seen before.

    After an hour of intense fighting, the volunteer company had extinguished the fire and saved the secret formulas. The joyous chemical company president announced that he would double the reward to $200,000.00 and walked over and personally thanked each of the volunteers.

    After thanking each of the old men individually the president asked the group what they intended to do with the money. The truck driver looked him right in the eye and said: "The first thing we're going to do is fix the blasted brakes in that truck!"
